Print a copy for your internal use. You can use the annual report document as a guide for completing the Excel Template.
Submit your Home Health Agency Annual Report electronically using the Excel Template. Send the report via email to: mahc@homecaremissouri.org

NOTE: Please change the name of the Excel Template to include the name of your agency. For example if your company’s name is the “Cool Home Care”, name your file “Cool Home Care” when you submit to MAHC.
