Home Health Agency Annual Report
The Home Health Annual Report is Due Every January for Home Health Agencies that Provide Home Health Services in Missouri.
How to Complete and Submit
- 2009 Excel Spreadsheet Template.
- Save the Excel Spreadsheet to your computer.
- The directions and definitions are contained inside of the Excel file.
- Directions/definitions will display automatically in each cell where directions are required.
- Please read the instructions on the “Instruction” page before proceeding to complete the template.
- Submit your Home Health Agency Annual Report electronically using the Excel Template. Send the report via email to: Melissa@homecaremissouri.org
NOTE: Please change the name of the Excel Template to include the name of your agency. For example if your company’s name is the “Cool Home Care”, name your file “Cool Home Care” when you submit to MAHC.
2008 Home Health Agency Annual Report Data

Home health agencies in Missouri must provide annually certain information to the Unit of Home Care and Rehabilitative Standards of the Missouri Department of Health and Senior Services. Missouri Alliance for Home Care’s Annual Report Data Book and Disk are compiled based on that information provided by home health agencies.
THE BOOK – The Home Health Agency Annual Report Book does not compare agencies to one another; rather, each page is organized alphabetically by agency. This data provides a valuable tool for home care providers to evaluate services, patient populations and identify growth opportunities.
THE CD or EMAIL ATTACHMENT – Provides you with access to the data to sort, compare and analyze in any way (i.e., visit ratio comparison of for-profit versus not-for-profit agencies in various areas of the state; visits per admission benchmarks, etc.). The CD or E-Mail Attachment is a great way to access information on market share by county or diagnosis information.
